District 11

Hello, 

District 11 is in the beginning stages to plan a Recreation Leader training for our district. I'm trying to locate a "Call For Sessions" form that we can use for people to submit a training they would like to host at the event. We seem only have the PDF copy of our Region 4 Mini Conference application but I need a working document that can be edited. 

If you have something that we could use, can you please email it to madams@chinohills.org? It would be greatly appreciated.
